Persons aged 22 or older who have an intellectual and/or physical disability may apply for service at the Cosmopolitan Learning Centre by completing the prescribed application form (please note there is no age restriction for the Supported Career Initiatives are). The Executive Director and the designated member from the Admissions & Review committee oversee and review all applications that the Centre receives.
If requirements are met, the applicant would receive a written notice stating that they have been placed on the waiting list for services. When openings become available, applicants would then receive a letter of acceptance into the program. Following a letter of acceptance, the new applicant would be contacted to set up a planning meeting with themselves, their team members and a Program Coordinator from the Centre. Participant to staff ratios are taken into consideration when determining which area the new participant would be best suited and supported within the Centre. In the event that an application for service has been denied, the individual shall receive written notice.
The Cosmopolitan Learning Centre works collaboratively with the individual and their team members to plan a successful transition into the program. Transportation too and from the Centre is provided based on participant needs. Effective upon start date into the program, all participants are placed on a probationary period. Tours of the Centre are available to those who are interested in attending the Centre.
Program hours are provided Monday to Friday, 8:00am - 3:30pm (variance can occur). The Centre is closed for all statutory holidays, scheduled staff in-service days, as well as three weeks of scheduled holidays throughout the year.
Please be aware that the Participant Application is used to determine where the applicant would be best supported within the Cosmopolitan Learning Centre. Therefore, it is important to fill out all fields to the best of your knowledge and to include all applicable history. Applications are reviewed regardless of the level of health needs and/or behavioural challenges.
